PCAAT: Making access to primary care easier

Through a team-based approach, Refuge Newcomer Health, Shelter Health Network, HAMSMaRT, and CMHA Hamilton Branch will bridge gaps and facilitate timely care and access to primary care for those who face barriers.

PCAAT serves individuals aged 16+ or family units who are experiencing complex health and social needs related to trauma, substance use, social isolation, displacement, and mental health concerns.

Primary Care

We help you access the care you need — whether for acute or chronic health conditions. We offer complementary supports to help you overcome barriers like housing instability, poverty, or refugee status, and connect you with primary care that works for your life

Mental health services

Mental health counselling, peer support/advocacy, nursing (RN’s) and nursing services will allow clients to benefit from a coordinated, interdisciplinary approach specifically geared to the complexity of their circumstances.

Coordination with other services

PCAAT will advance coordination with a range of established services already available through the agencies connected to the Hub including footcare, community connectors, harm reduction, RAAM Nurse Practitioners, settlement and housing workers, and transitional care beds that may further support successful attachments to appropriate primary care settings.

Eligibility Criteria for PCAAT Referrals:

All referrals should consider the following

  • 16 years of age and over or part of a family unit
  • Be facing significant barriers to accessing a primary care provider
  • May include newcomers, and people experiencing homelessness, mental health, and substance use concerns

Mental Health Services

Working with the team at CMHA Hamilton, the PCAAT Mental Health Workers provide short- to medium-term services including crisis assessment and intervention, mental health counselling, person-centred care planning, and improved access to primary care and longer term supports in the community.

Peer Support Services

Working with the team at HAMSMaRT, the PCAAT Peer Support Worker provides support and access to care in the community through a lens of lived experience. Services include systems navigation, skill building, safety planning, support for substance use and mental health concerns, and connections to resources that best meet individual needs.

Nurse practitioners

The PCAAT Nurse Practitioners work with the Shelter Health Network and HAMSMaRT to provide primary care to individuals accessing those services.  The Nurse Practitioners are able to work in their full scope and complete assessment, treatments, and prescribing for patients that access their services.  They also provide links to other services via referrals and collaboration. 

Newcomer & Refugee Health

Working with the team at Refuge Hamilton Centre for Newcomer Health PCAAT provides client-focused, community-driven, comprehensive healthcare services to Hamilton’s new immigrant and refugee population. The primary focus is to reduce barriers to healthcare access with an emphasis on communication through appropriate interpretation, providing culturally appropriate care and advocacy in a community-based setting.
